Blog Update 01/17 – The 411

baby - ivf egg and sperm

My embryos are growing in a lab and awaiting Mommy to come get them and nestle them inside ! Tomorrow at 10:30 am, I have acupuncture and then at 11:30 am, Dr. Kiltz is going to transfer embryos to my womb.

Today, I got up as usual and started my injection and pill swallowing regimen.  I then learned that a friend, Comedian Joe Torry is flying in to Syracuse and performing tonight at Funny Bone Comedy Club and Restaurant at Destiny USA.   My mom and I went there this morning to see Will Packer and Tim Story’s new film Ride Along 2.   The movie was really good.  After the movie, we went to Cheesecake Factory for dinner.

Tonight, I am going to go back to Destiny USA for Joe’s comedy show.  It should be a lot of fun.  I tried to get my mother to come along, but she has declined to go.  She only likes a handful of comedians because she says they do a lot of cursing and the majority of their content is about sex or vulgar.  lol

In family news, my sister who lives in North Carolina and my little mini me niece drove up to my other sister’s house in Northern Virginia/DC area for the impending birth of our niece’s baby. My niece is finally having some contractions and her mucus plug came out last night or early this morning.  By the time I get back to the DC area, I will be a great-auntie !  Not sure I like that title… I am not even a mother yet…

This week is going to be a stressful one. My mother will get the results of her CT Scan on Thursday at her appointment with her Oncologist.  We are praying that she gets an all clear, despite the fact that she is acting like she expects them to find something.

I would love to hear from you ladies as to what you feel I should do if I have more than two embryos tomorrow.  I am torn still as to how many to transfer back.  As far as I know, I have three, three day embryos.  I am not sure if I should transfer two and hope for the best, or transfer all the embryos.  My dilemma is, if I transfer three, then I will have no embryos left and will have to do another cycle with another donor since my current donor is no longer cycling.  I do have another donor that I like, but she does not look like me and is much lighter in complexion than I.  If I transfer two, then I would have one to freeze, if it is of quality to freeze, and transfer later.  The only concern there is that the embryo may not be good enough to freeze and also that even if frozen, does not guarantee that it will survive the thaw.

If I transfer all three, and none of them stick, then I have to start all over again.  The new donor would have to do a fresh cycle instead of a frozen one.  There are no donors in the frozen bank that I like.

Why does this have to be so darned complicated?   If only all of the eggs would have made embryos, then I would have two transfers worth of embryos or even three if I did two at a time.  Then again, that would all depend on them making it to freeze and thawing properly.

I definitely want twins, however, I do not want triplets.  My mom wants me to have triplets… not sure why that is even a thought in her head, given how she knows I have no tolerance for a lot of foolishness and just a little bit of patience. Two will be a lot of work, but I am mentally prepared for it.  I just can not see myself with three kids. Whoa !

Prayers needed tomorrow and please weigh in with your thoughts ~

Blog Update 01/14 – The 411

I am as nervous as hooker in line on judgement day !  Tomorrow, my eggs will be thawed and fertilized !!  Please pray that all seven … okay I want all seven to do their thing, but I will accept six thaw and fertilize and grow into beautiful babies !!!

Please send up a prayer at 6 am for thawing and at 1 pm at fertilization !

Tonight is my last Lupron injection.  Tomorrow, I inject Lovenox, Neupogen, Delestrogen and PIO in the morning.  I start the Endometrin suppositories three times a day along with the Estrace.  I also have to use the Cleocin suppository at bedtime.  My poor little hoohah is going to be more irritated than it is right now.  I have hydrocortisone and Lamesil on deck !

I am about to whip up some juicy burgers for dinner and my sister made twice baked potatoes.   I am going to the screening of Ride Along 2 tonight with my mom and sister.

Tomorrow, I have to get up early and go to the hospital for blood work and a sonogram.

Good night !  xoxo

Update 1/09 – The 411

Today is my second day on z pack and I am still struggling to keep food down.  My stomach has at least stopped burning.  I woke up at 4:00 am hungry for the first time all week and guilted my mom and sister into taking me to Waffle House for grits and toast.  I ate and it came back up a few hours later, if that long.

I was incredibly touched that Dr. Kiltz contacted me to check on me yesterday evening.   Nothing like having doctors who care about you! !

I am feeling better than I was yesterday and am now back in the bed and resting.  It is sad that the majority of restaurants only serve cream based soups in lieu of broth based.  I have wanted chicken noodle soup all day.

My sister is going to make Pho tomorrow for me.

I hope everyone is having a great weekend !!

IVF #5 – Here We Go- The 411

ivf #5 think positive

I am nothing if not determined !

I spoke with my RE, Dr. Robert Kiltz, who owns CNY Fertility Center and Spa yesterday and he called me again today after reviewing my chart in more detail.  He was very receptive to my concerns and took responsibility for the embryo glue not being sent to Albany office for my transfer and we also discussed putting in place a protocol that requires pre-preparation of a medium which will culture immature eggs.  This is not a standard practice, but something he has been exploring and is going to put in place so when patients have immature eggs, the proper culture is there to place them in to give them a fighting chance of maturing.  It is called IVF Maturation…

Long story short, he reviewed my chart and is willing to do another OE IVF using a different protocol and different medications.  I will be doing the Agonist / Antagonist/ EPP protocol.  This is the protocol that Dr. Sher mentions for poor responders and advanced maternal age women.  It is a protocol that CNY has been tweaking and using for women which has shown a lot of promise and success where other protocols have failed.   Dr. Kiltz was very warm and receptive and assured me that we will get this done one way of the other.  He is willing to take risks that a lot of other doctors are afraid to do until they show 100% promise that it works.  Dr. Kiltz believes that if it is not harmful and can be helpful, lets give it a shoot !

I am starting BCPs (Desogen birth control pills) tomorrow as part of the protocol.  The birth control pills are taken for 10 days or so then overlapped with Lupron for 5-10 days.  Once I get my next period (27 days from today), then I start the antagonist and estrogen priming part of protocol, in which I take Ganirelix daily, Neupogen injections daily, Delestrogen injections twice a week (for 2 wks), and then add Follistim (150 units or more) on CD 6 and then we will add Menopur (37.5 to75 units) at the very end for 4 or more days.  Menopur add testosterone to the ovaries, which causes the eggs to not develop properly and then you end up with a cycle with lots of follicles and few eggs and also a lot of immature eggs.  This has happened the last three cycles.

After speaking with Dr. Kiltz, I feel a lot better and am grateful that he listens to patients, takes a look at your chart and does not put patients in a cookie cutter protocol.  He stated they are working hard at revamping the centers so that all of the centers are run the same and seamlessly and they are working on implementing new protocols that are working to improve the live birth rates.

I have referred several advanced maternal age patients lately and I just got word that two of them are pregnant on the 1st time at CNY !!  They had had failed cycles at their other centers.   One of the patients is 44 !!  Woot Woot !  I am so excited !

Now, if we can just get ME pregnant so I can so do some other things LOL.

I did ask if I should consider changing donors, and Dr. Kiltz said I did not have to because it was egg issues and not sperm issues.  I am still thinking about buying the access to the cryobank site and look at the donors they have on there. I am certain that they have added a ton of new donors and some of the ones that were new when I started may be “proven” by now.  It has been a year, so I am leaning towards at least exploring different donors.  I just happen to really be stuck on this one ! 🙂  He is soooo the male version of me and it took me forever to decide on that one…

IVF #4 – 6DP3DT- The 2WW – The 411

baby - ivf 4

At 6DP3DT, this morning, I had to go to LabCorp for blood work ordered by my Reproductive Endocrinologist.  They ordered progesterone (which is 35) and estradiol (which is 1207), and they also ordered a TSH and Vitamin D, which has not been posted as of yet.  Dr. Kiltz is now requiring blood draws at retrieval, transfer, 4-7 days after transfer and then beta 7 days later.  They are charting something to try to improve BFPs.

I am now dealing with insomnia !  I am not sure why or if it is a result of the progesterone or the embryo taking root.  I am so sleepy off and on throughout the day.  I climbed in bed this morning and took a three hour nap !

I got up, went downstairs and mixed up meatloaf.  My mom has been bugging me about making one for weeks and my nephew in California LOVES when I make it, freeze it, and ship him one.

Meatloaf making 1

Meatloaf making 2

Today’s dinner is going to be meatloaf, mashed potatoes, string beans and yeast rolls.  🙂

IVF #4 – Precycle CD3 – The 411

baby - ivf 4

Yesterday, I had my first precycling baseline appointment and bloodwork.  The transvaginal ultrasound shows that I have six follicles on my right ovary and three on the left. My blood work is all normal. I am now scheduled for my hysteroscopy with biopsy on January 9th at Lenox Hill Hospital, which will be my cycle day 20. I am continuing with weekly acupuncture as well.

Last night, Dr. Kiltz returned my call regarding the next steps of my protocol.  I spoke to Dr. Grossman earlier yesterday morning and he said we would defer to Dr. Kiltz’s protocol.  Dr. Kiltz had to look over my chart and figure out what he wanted to tweak for this protocol.  He stated he wanted to shake things up a bit.   Today, I got my precycle instructions for IVF#4.  I will be taking my normal supplements and prenatal vitamins until January 7th.  Starting on the 7th, I am to start taking Estrace tablets twice daily for seven days. Then I am to call with my next period to do a day three monitoring/ baseline appointment and start stims.  I will not be taking Estrace for three weeks and Endometrin the last week and starting Lupron like I did before.

I did discuss with Dr. Kiltz my desire to do a double trigger this next cycle, which is proven to mature eggs.  The hcg trigger shot is taken 36 hours prior to retrieval as normal.  The next morning, I will go in for blood work to test the hcg level and then that night, I will either do a lupron trigger of over 20 units or another hcg trigger.

I will be doing Saizen, 300 Gonal F, 300 Menopur, 5 Lupron (I assume) along with Synthroid and prenatals during stims.  After egg retrieval, I will add neupogen, lovenox, prednisone (20mg) and weekly intralipid infusions. I will be doing a two or three day transfer of all embryos this time instead of waiting until five day.

Merry Christmas to you all !
